Mayo Clinic Health System Announces Six Regional Clinics will Close; Care Transitioned to Nearby Locations

Mayo Clinic Health System has announced that six of its regional clinics will transition services to nearby locations by December 10, 2025. The affected sites include Belle Plaine, Caledonia, Montgomery, Northridge (North Mankato), St. Peter, and Wells. Mayo leaders say the move is part of a broader effort to strengthen care delivery across its communities while ensuring patients continue to have access to reliable, high-quality care.

Patients at the closing clinics will be able to continue seeing their current providers at other Mayo locations whenever possible, or will be connected with another trusted Mayo provider. Those with appointments already scheduled after December 10 will be contacted directly with details on their new care location. To help with the transition, Mayo has set up a dedicated phone line at 651-267-5500 and is sending letters and portal messages with additional information.

Mayo officials acknowledge that travel is a concern for patients and say they are committed to helping individuals transition to the closest, most convenient site available. To reduce unnecessary travel, Mayo will continue to offer same-day in-person appointments as well as its Primary Care On Demand service, which provides 24/7 access to providers by video for routine and urgent needs. More details on provider locations and transition plans are expected within the next month.
